New Patients

Welcome. Here is how to start.

Thank you for choosing our office. Please arrive at least 30 minutes early for your first visit so there is time to complete registration. Completing your new patient packet ahead of time makes check-in faster.

We believe good medical care begins with open communication. We will speak candidly with you about your symptoms, your diagnosis, treatment options, and our recommendations.

Bring to your first visit
  • Your insurance card and a photo ID
  • A list of your current medications and dosages — or simply bring the bottles
  • Results of any related medical or laboratory tests

Established Patients

Keeping your care current.

Patient forms — such as a records release or HIPAA authorization — are available under Office Policies & Forms. If you need one for your visit, please print and complete it in advance, then bring it to your appointment.

Please arrive about 15 minutes early in case any forms on file need updating.

Finance & Insurance

Medical care that is clear and affordable.

We are committed to keeping the cost of your care clear. If you have questions about what a visit, test, or procedure may cost, ask us — we will help you estimate charges whenever we can. Payment and insurance co-pays are due at the time of your visit.

If you have questions about payment, a member of our staff can review options with you and help find an arrangement that fits. If special arrangements are needed, please speak with our office manager before your visit.

Before your appointment, please check with your insurance company that we are in your plan's network.

We will file your insurance claims for you. If we are not in your plan's network, out-of-network costs may apply.

Prescription Refills

Refills, made routine.

You can request medication refills through MyIHChart or by calling the office and leaving a message with our staff. Please allow three business days for refills to be completed.

Refills on controlled medications are handled during office hours only. You may be required to sign a controlled substance agreement.
